The "number" and "image" icons in the top-left corner are part of the Photoshop Extras option for handling Slices.
To disable: 1. Go to Edit > Preferences > Guides, Grid, & Slices. 2. Uncheck the "Show Slice Numbers" option.
This extras icon has been present in my app for months, and I didn't use PS often enough to opt to troubleshoot removing it. Thanks for your S.O.S. -- I may have been trapped in the "just press CTL+H each time you open a file to hide it" hell for a while!
