I think you can still hold the yarn between your thumb and the cast. That's how I hold the yarn (yeah, I do it "wrong" and don't wrap it around any fingers).
Then wedge the right needle under your arm or between your knees.
I'm thinking that might work, as would learning to knit continental, but I do a lot of hand twisting when I knit (that's proabaly "wrong" too) and I can't do that at all right now. :-(
